Your submission must include all the items in the following checklist. It is recommended you check you have digital files on your computer for each of the items below. You can modify and revisit your submission at any time prior to submitting the entry.
- Copy of your qualification certificate (if completed) OR a statement of attainment for all units completed to date

- Copy of the most recent academic records or statement from training provider
|
- Copy of any additional awards, prizes or achievements
|
- Three photos supplied of the apprentice taken on-site performing day to day work activities as a landscape orientation high resolution image (taken on a camera, not on a phone)
