Your entry submission must address all of the criteria below along with supporting documentation where applicable:

1. Outline the work health & safety management plan for your business.

2. Explain your procedures for monitoring and reviewing your work health and safety performance.

3. Describe how your overall safety procedures are followed on site including site specific hazard assessments, use of signage and procedures.

4. Describe how initial work site assessments are carried out to identify hazards that may affect the building process and explain how new hazards or risks are dealt with and communicated to workers and contractors. Provide examples.

5. Describe how you ensure sub-contractors will work safely on your jobs. Please provide examples.

6. Describe how your business keeps employees and subcontractors up to date with new work health & safety information, procedures and safety solutions or situations where you perform work in other jurisdictions where different laws may apply.

7. Reflecting on your previous 12 months of safety performance, identify how you have managed changes in safety performance and new hazards or risks. Provide examples which highlight your achievements. Examples may include injury or incident data, case studies, descriptions of a new system(s) implemented and/or descriptions/profiles of a new product being used.

8. Describe any industry contributions your business has made in relation to work health and safety. Provide an outline of your contributions (forums, committees, working groups, industry projects, safety innovations).

9. Describe the steps, if any, you will be taking in the future to further enhance or improve your safety culture.
